Fentanyl is a powerful synthetic opioid. In illicit markets it is often mixed into other drugs—sometimes without the buyer knowing. A very small amount can be deadly.

Why synthetics changed the crisis

Unlike many plant-based drugs, synthetic opioids can be produced and distributed at enormous scale. Potency varies. Pills may look familiar and still contain lethal doses. People who used substances for years describe a different, more dangerous landscape than the one they knew before.

• "Just this once" can still be fatal when potency is unpredictable.


• Youth and casual users are not immune; curiosity and social pressure still meet a poisoned supply.
